The posts are offered as part time or full time hours offering a fantastic work life balance, with a shift pattern which means you only work one weekend per month.

We are a friendly and supportive team, looking for highly motivated RNs who possess strong communication skills. Your role will be pivotal within the local community by making a difference to our patients, delivering a high level of care in their own home. 

As a Community Nurse no day is the same and you will be met with patients of varied needs, from wound care to end of life care. Giving you the opportunity to develop coupled with a supportive environment  with opportunities for further career development

A typical week

Monday to Friday ; 8.30 - 16.30

1 weekend every 4 weeks (Time off week)

Early Shift 07:00 - 15:00        Late Shift 11.00 - 19:00

 Band 5 - Salary £27,055 - £32,934 per annum

Main duties of the job

 

Further information about this opportunity:

  • You will visit several patients daily in their own homes 
  • You will provide high quality nursing care to a range of patients with varying healthcare needs.
  • Provide a high standard of holistic assessment and care planning.
  • You will be expected to provide wound management, health promotion, medicines management, end of life care.
  • Area : Thatcham, Chieveley, Compton, Chapel Row, Kintbury, Woolton Hill, Hungerford and Lambourn

 Detailed job description and main responsibilities

The 3 “must haves” for you to be considered for this role:

  • A passion for community nursing, that matches our passion to provide excellent healthcare for patients within their own homes,
  • Either a qualified adult nurse or currently in your last year of training (with NMC registration)
  • Hold a full UK Driving Licence and have use of a car for work purposes

Previous Experience

Essential criteria
  • Experience of undertaking general assessment by utilising evidence based care
  • Community Nursing Experience
  • Evidence of effective team working
  • Ability to demonstrate and show evidence of effective clinical skills
Desirable criteria
  • Experience of teaching relatives and carers across all areas

Knowledge Sills & Abilities

Essential criteria
  • Well - developed written & verbal communication skills
  • Ability to assess, plan, implement and evaluate care.
  • Ability to identify risks and act accordingly with regard to safety of oneself, patients, and others using the appropriate aids.
  • Able to demonstrate use of own initiative and ability to work alone
  • Ability to demonstrate organisational skills
  • Time Management Skills
  • Ability to demonstrate an understanding of LTC's and the concept of self - management
  • Ability to participate in standard setting and audit
  • Computer literate

Additional Requirements

Essential criteria
  • To work bank holidays / weekends or unsocial hours to meet the needs of the service
