We are pleased to be able to offer a part time 28.5 hours per week band 3 Multi-therapy Assistant role working in the Wokingham area of Berkshire. We are a community team covering from Twyford to Finchampstead. We are an integrated team including Occupational Therapists, Physiotherapists, and Therapy assistants. The service supports multiple pathways including Community Reablement, Rapid Response, Falls and Community Physiotherapy. This is an exciting time to join BHFT, as we develop services to work in an integrated way within the community in Wokingham and across Berkshire West.

The successful candidate will be required to provide high quality, responsive services working as an integral member of the Intermediate Care Team and wider Integrated Community team. You will work closely with social services and voluntary organization for adults with a wide range of physical impairments and mental health conditions.

The successful candidate will work autonomously under the guidance of the Care Coordinator and Therapists having a clinical responsibility for a designated caseload as well as supporting the delivery and monitoring of care visits and therapy plans delegated from the clinicians.

 Salary - £21,396 - £22,866 per annum, pro rata (incl of HCAS)

Main duties of the job

 

•    Support all intermediate care pathways to promote the timely assessment and management of referrals with multiple co-morbidities and rehabilitation needs.
•    To support band 3 therapy assistants of the Intermediate care team with clinical development.
•   To support the triage of referrals into the intermediate care team reviewing service user needs and sign posting to other services when indicated.
•   Hold a caseload and work autonomously under the guidance and support of the therapy assistant coordinator and therapists.
•    Work flexibly to ensure urgent referrals are adequately managed
•    Supporting colleagues in the delivery of compassionate, safe and structured care
•    Provide guidance and appropriate delegation within the team to band 3 colleagues.
•    Working pattern + Service operating hours

Intermediate care team is a 7-day service which operates from 08.00 – 22.00hrs with Early (8:00-16:00) and Late shifts (14:00-22:00). You would be expected to be able to cover all aspects of the shift pattern. Shifts are rostered 3 months in advance and shifts/days off etc can be requested in advance. Flexible working requests can be discussed and considered at interview. 

The 3/4 “must haves” for you to be considered for this role: 
 
•    NVQ 3 or Equivalent experience.
•    Ability to demonstrate interest in working with older people.
•    Ability to demonstrate the needs of patient/client with long term conditions
•    Ability to Drive & have use of a car
